The tax credit system remains deeply flawed, in spite of recent attempts to improve the system, the parliamentary ombudsman will say on Tuesday.

Ann Abraham will also say it is “fundamentally unfair” that Revenue and Customs (HMRC) has discretion over whether or not to recover the overpayment of tax credit.
