Carolyn Merritt, chairwoman of the US Chemical Safety and Hazard Investigation Board (CSB), said BP's fatal accident at a Texas refinery last year underlined the need for US regulators to be more proactive in preventing workplace disasters.
She warned that government agencies must be more rigorous in their inspection of hazardous processes "before a catastrophe occurs. The current enforcement system simply can't deter safety violations by corporations, large or small."
